These documents were developed through the collaborative efforts of the Marion County Court, the State’s Attorney, the Public Defenders, Guardian Ad Litem and DCFS and private agency supervisors.  These forms are used only in Marion County and are not part of the forms approved by the AOIC or Conference of Chief Judges.  They may be used and modified by any county that may benefit from them.
